Survey Deadline is November 7, 2003.

NAMI (National Alliance for the Mentally Ill) is conducting a national survey of parents and caregivers of children and adolescents with AD/HD.

The survey is brief and can be completed in a short amount of time. Parents and caregivers who would like to participate can access the survey online at http://www.nami.org/adhdsurvey.

If you do not have Internet access and would like to participate in NAMI's survey, please contact Belen Assusa in the NAMI national office at 703-600-1110. A copy of the survey will be mailed to you.

The deadline for the survey is November 7, 2003.

The NAMI Child & Adolescent Action Center will use the information gathered from this survey in its advocacy work to improve access to services and appropriate treatment for children and families affected by AD/HD.
