You are invited to participate in a survey
that is being conducted by the Developmental Disabilities
Institute at Wayne State University. This survey will help the
Institute in developing its next 5-year plan.
The Institute is Michigan’s Center for Excellence in
Developmental Disabilities and, as such, is part of a network of
centers throughout the United States. The Institute’s mission is
to contribute to the development of inclusive communities and
quality of life for people with developmental disabilities and
their families through education, community support, research,
and dissemination.
To help in generating ideas, we have a brief survey that will
take less than 10 minutes to complete. You can complete it on
line, or you can download a paper version of the survey. Feel
free to distribute this on line survey link, as well as paper
copies of the survey, to anyone in Michigan who has a
developmental disability, is a family member of a person with a
developmental disability, or a service provider, advocate or
policy maker.
To complete the survey on line, click on the link below. If the
link does not take you to the survey, copy and paste it into
your browser.
