The Centers for Medicaid and Medicare
Services has accepted and awarded funding for a Michigan’s Real Choice
Systems Change Family-to-Family Health Information Center grant
proposal that was submitted by the Parent Participation Program (PPP)
in collaboration with Citizens to Uphold Special Education (CAUSE),
the Association for Children’s Mental Health (ACMH), New Center
Community Mental Health Services, and other partners.
Purpose: The purpose of The Michigan Family-to-Family Health
Information and Education Center is provide information, education
parent-to-parent support and decision-making opportunities to all
families of children with special needs by expanding and improving
Parent-to-Parent information, support and education activities to
underserved families.
Goals and Objectives: Improve access to quality care and
supports for children with special needs in their communities by
empowering families.

Provide information
to increase access to and utilization of coordinated, ongoing care
in a medical home for parents of children with special needs. |

Provide information to increase
access to and utilization of health care and public health
insurance of families with CSHCS. |

Provide outreach,
training and parent-to-parent support to families of children with
special health care needs especially those who receive special
education and early intervention services. |

Increase and
coordinate the dissemination of information on resources and
services to families of genetic conditions and birth defects. |

Increase and
coordinate the dissemination of information to families of
children with mental illness. |

Provide information
on transition to adult health care, work and independent living
for CSHCN and their families. |

Ensure families with
CSHCS develop effective partnerships with service providers/health
care providers and satisfied with the information and services
they receive. |
Methodology: To reach these
objectives, PPP will work with a host of family, community, state and
national partners to enhance our existing network resources and expand
our collaborative efforts for outreach, training and parent-to-parent
support. Partners will assist PPP in designing effective health,
health care coverage, medical home and transition information,
education and training materials to supplement our existing curricula
to help families (a) obtain information so they can access the
services necessary to keep their children at home in their
communities; (b) know what health care coverage options are available
to pay for medically-necessary services for their children; (c)
identify a “medical home,” (e) receive emotional parent-to-parent
support; and/or (f) help prepare their youth for adulthood. Our
lending library will be enhanced with relevant information and
dissemination strategies and new materials will be developed and
integrated with existing resources. For example, a health care module
in the CSHCS On-Line Course and a health care coverage page in the
Watch Me Grow Calendar. Intake, referral and follow-up will be
improved by the use of new software. |
