The Family Center for Children and Youth with Special Health Care Needs (CYSHCN) (Formerly PPP) details support offered to families through the Family Phone Line, Family Support Network, the Relatively Speaking Conference, and Conference Scholarships.
 
Family Guidance: Transition Planning for Youth (PDF)
Provides guidance to families preparing their youth for transition: independent living, health care skills, health care coverage, education and employment.
 

Foster Youth in Transition
Offers valuable information on education, employment, finances, housing, transportation, legal assistance, health and wellness, pregnancy and parenting, youth with disabilities, social life, being a volunteer.

 
The Michigan Transition Resources Listserv
Designed to provide educators, parents, families, and community members with a forum to exchange information on Transition services in the state of Michigan.

Resource Center for Parents of Children and Adolescents with Depression
