Transition for Youth: Health Care Coverage and Service Programs

Access to Long-term Care - Know Your Options
Downloads a brochure which offers information on Medicaid Programs that provide long-term care services and requirements for eligibility. Numerous home and community based services, long-term care resources and other resources are highlighted in a including the Michigan Medicaid and Medicare Assistance Program, Home Health, Home Help, Physical Disability Services (PDS), Home and Community Based Waiver for Elderly and Disabled Persons (MIChoice Program) and the All Inclusive Care for the Elderly (PACE) offered by the Michigan Department of Community Health to eligible persons.
