 Special Needs Daycare Survey

The following is a survey for generating support for opening a special needs daycare setting.

2) Do you, or have you in the past used your local community mental health services?

3) If answer 2 is yes, did they adequately meet all your needs?

4) Do you have problems finding qualified, dependable staff to care for your child in you home?

5) If there was a special needs daycare to fit the need of your child would you take advantage of it? Ex: transportation options, nursing and direct care services?

6) If funding was available through federal programs, state programs, or grants, would this make the facility more accessible to all families?
